
Quality improvement depends on closing the loop
Introduction

Root cause analysis is only one step in the quality improvement cycle.
Teams also need to know whether the corrective action they implemented actually reduced the issue, and whether the fix held over time. Without that verification, the same problem may return under a different part number, shift, line, or plant, and the connection to the original issue may not be recognized immediately.
Solving a quality problem without verifying whether the solution worked is one of the more common ways quality costs compound across manufacturing organizations. The same root cause may be investigated more than once because the first corrective action was never fully confirmed to have held.
Closing the loop means connecting five steps into a repeatable cycle: detection, investigation, corrective action, verification, and prevention of recurrence. Each step depends on the one before it. Verification without thorough investigation produces fixes that may not address the real cause. Prevention without verification produces guidance built on assumption rather than evidence.
